Community Health Worker for Health Equity and Social Connectedness for Older Adults Program

Location: Española, NM - Employer: Bridge to Health NM

Position Type: Contract through June 30, 2025, then probable transition into full-time with benefits starting July 1, 2025.

Bridge to Health NM is seeking a Community Health Worker (CHW) to support the Health Equity and Social Connectedness for Older Adults Program in collaboration with the UNM Health Sciences Center. CHWs play a vital role in building relationships with clients, providing education, organizing events, and advocating for improved community resources. This position requires cultural competency and a deep understanding of Northern New Mexico’s communities. Travel around Rio Arriba County is essential.

Key Responsibilities

  • Conduct assessments to identify social needs affecting older adults.

  • Provide individualized navigation to healthcare, social services, and community resources.

  • Offer training, workshops, and organize community events to promote health and social well-being.

  • Work closely with clients, families, and partner organizations to address SDOH barriers such as food insecurity, transportation challenges, and access to healthcare.

  • Maintain detailed records and reports to track client progress and service effectiveness.

Qualifications

  • CHW certification is NOT required—training and certification will be provided.

  • Spanish language proficiency is a plus.

  • Must have a strong understanding of the culture, history, and needs of Northern New Mexico communities.

  • Ability to connect, communicate, and build trust with older adults and diverse populations.

  • Strong organizational, advocacy, and problem-solving skills.

  • Reliable transportation is required.

Compensation & Benefits

  • Independent Contractor through June 30, 2025

  • Probable transition into full-time employee on July 1, 2025
